Immunizations By Appointments for Berkeley County High School Students 6/2/26 & 6/3/26

Appointments are now being scheduled for June 2nd & 3rd to take place during the Free Sports Physical event at WVU ME- Berkeley Medical Center.

• Parents will be notified of tentative time and day of schedule appt.

• Appointments will be scheduled based on the health department receiving all necessary information.

• Parent/guardian must be present with the student for their appointment.

  
  

  Student Immunization Record & Insurance Information Required

• If the student is not a Berkeley County Health Department patient, the student’s immunization record must be sent to the health department prior to the appointment.

• Insurance information must be emailed, faxed, or dropped off to the health department in advance.
