Comments must be received prior to 05/18/2022.

Copy Charge per Page $0.50

Plan Review/Evaluation Fee (Wastewater Programs) $75.00

Environmental Program Letter w/o visit $25.00

Port-a-pot review letter $25.00

Plan Review Fee (Other Programs): $75.00 (Mobile and/or 0-20 Seats)

$125.00 (21-50 Seats) $225.00 (51-80 Seats)

$325.00 (81 Seats and above)

Research Fee $25.00 per parcel/lot

Re-inspection $150.00 per visit

Septic or well permit renewal Fee $50.00

ServSafe manager class $175.00

State food safety proctor fee $75.00

Water Test (includes lab cost) $95.00 each visit

Shipping for all specimens not covered by State $ Current Rates

Only Exceptions to fees shall be for official non-profit organizations that receive prior approval from Administrator or Health Officer.

Clinical Fee for Service Schedule

Children/Adult Vaccines (Non-VFC) Charged at the rate of 20% above cost.

Administration of Vaccine (Per vaccination) $20.00

Copy Charge per Page $0.50

Research Fee for records $25.00 per individual patient

PPD Test $20.00

Consultation for Travel Vaccine $50.00

Women’s Health Programs covered under WV State Contract shall be charged according to approved “Program” fee schedule.
